We have provided the forms/paperwork that are necessary to play football at Berkner HS.   The UIL Physical and related forms must be completed and turned in to the Berkner Athletic Training staff before participation in any workouts can begin.  Student insurance is also available for purchase,  but is optional.   To download any form, simply click on it below and it should be available to print as a PDF file.
